Blind Squirrel Golf Network ("BSGN") creates entertainment-based golf content that may be published on YouTube, social media platforms, websites, and other digital or promotional channels.

By agreeing below, I voluntarily consent to being photographed, filmed, and/or audio recorded while participating in Blind Squirrel Golf Network activities.

I give Blind Squirrel Golf Network permission to record and use my name, approved biographical information, image, likeness, voice, interview responses, golf performance, and other recordings made during my participation.

I understand and agree that BSGN may edit, reproduce, publish, distribute, display, and otherwise use this content for entertainment, promotional, advertising, and commercial purposes, including monetized content, without additional approval from me or compensation to me unless otherwise agreed to in writing.

I understand that BSGN content may include humorous commentary, jokes, graphics, statistics, replays, editing, sound effects, and commentary regarding my golf shots, decisions, reactions, statements, and performance.

I understand that edited content may differ from the order or context in which footage was originally recorded, and that once content is published online, BSGN may not be able to control copies, shares, reposts, or other third-party use.

I understand that submitting this form does not guarantee that I will appear in a BSGN video or other content.

To help BSGN keep upcoming videos, concepts, and content a surprise, I agree not to publicly post, share, publish, or disclose unreleased BSGN footage, photographs, graphics, episode details, or other non-public information I learn through my participation.

I also agree not to publicly announce my participation in BSGN or share behind-the-scenes photographs or videos from filming until BSGN has publicly released my appearance or gives me permission to do so.

This does not prevent me from privately discussing my participation with my spouse/partner, attorney, tax or financial adviser, or another person who reasonably needs to know, provided I ask them to keep unreleased information confidential.
